Residents who are on a private water system on Trout Lake are warned not to drink the water after a tanker truck overturned in a single-vehicle crash on Highway 63 north of Silver Lady Lane.

The crash at about 8:30 a.m. caused fuel and a load of formaldehyde the truck was carrying to spill directly into the lake.

The North Bay and District Public Health Unit released a statement saying the municipal drinking water supply is not in danger.

Anyone using a private water system must find another water source until further notice. The public is advised not to swim in Trout Lake or use the water for pets or livestocks.

ITTSFIELD -- A Berkshire Med ical Center laboratory was evacuated for several hours Wed nesday afternoon following a chemical spill.

A hazardous materials team from the Pittsfield Fire Department responded to the hospital after a lab worker accidentally dropped a container that held the chemical formalin, which is used to preserve laboratory specimens, according to BMC spokesman Michael Leary.

"Due to the caustic nature of the material, it may cause severe eye irritation, skin irritation and it's harmful if swallowed," said Deputy Fire Chief Bruce Kilmer. "So we knew we had a definite hazmat incident, and that we were going to have to make an entry into the lab to clean it up and make air levels safe."

No injuries were reported, but the two BMC employees who were closest to the spill of less than half a gallon of formalin were taken to the hospital's occupational health center as a precaution.

Formalin, a formaldehyde solution, is considered a skin, eye and upper respiratory irritant and a probable human carcinogen after prolonged exposure.

A Winnipeg hazardous materials team spent much of Wednesday afternoon cleaning up a small formaldehyde spill at St. Boniface General Hospital.

Hazmat units were called to the hospital at around 2:45 p.m. CT, after staff in the shipping and receiving area noticed a three-litre container of formaldehyde was leaking, according to the Winnipeg Regional Health Authority.

The receiving area was evacuated and nearby air vents were blocked due to fumes from the formaldehyde leak, but patients are not at risk, the health authority said in a release.

The hospital blocked all "non-essential foot and vehicle traffic" around the receiving area so the hazmat teams could clean up the spill.

As of 7 p.m., the hazmat teams have completed their work, but hospital staff are still cleaning up the area.

Sixty people were evacuated from a major hospital in Melbourne's north-east as firefighters struggled to contain a chemical spill.

Workers on the entire sixth floor of the Harold Stokes building at the Austin Hospital in Heidelberg were forced to leave the premises about 9.30am in the wake of a formaldehyde leak in a pathology lab.

Formaldehyde is a pungent-smelling and highly toxic gas that can cause death if inhaled in large quantities.

Hospital spokeswoman Taryn Sheehy said that no one was injured and no patients were affected by the fumes. "It wasn't a serious breach ... essentially it's an incident we have dealt with," Ms Sheehy said.

DALTON, Ga. (AP) - Authorities say a FedEx semitrailer hauling chemicals containing formaldehyde overturned on an interstate near the Georgia-Tennessee line, forcing the highway to close for a time as crews cleaned up the chemical mess.

Sgt. John Burnette of the Georgia State Patrol says the truck, which was hauling two trailers, hit a guardrail and overturned around 5 a.m. Thursday. The wreck happened on Interstate 75 about 30 miles south of Chattanooga.

Burnette says hundreds of small bottles of sterilization fluid, each containing a small amount of formaldehyde, spilled from the truck.

A chemical used during root canals spilled in the cupboard of a Whitehall Township dental office Thursday morning, forcing the evacuation of about a half-dozen people, fire officials said.

A female employee was overcome by the toxic fumes, but was treated at the scene and released, according to fire chief Robert Benner. About six other people in the Dental Dreams, most of them employees, were evacuated around 11 a.m, he said.

The chemical was neutralized and disposed of properly, along with the cupboard.

WOOLSTOCK - A hog facility south of Woolstock was declared a complete loss Wednesday night after a fire burned the structure to the ground. Complicating matters for firefighters was the potential for hazardous chemicals inside the building which were used to fumigate the structure earlier in the day.

The Woolstock Fire Department arrived at the scene, 1929 Stagecoach Road, at about 3:30 p.m. and found a fire smoldering.

"When I got here, there was a bunch of black smoke," said Ethan Anderson, Woolstock fire chief. "We were told the building had been fumigated and it was filled with formaldehyde so we couldn't get into the building."

The hazardous materials team from the Fort Dodge Fire Department was called in to assess the danger. Firefighters were advised not to go inside the structure or use any water until Hazmat arrived, Anderson said.

"We got called for a chemical reaction from a fumigation process that was giving off vapors," said Capt. Steve Hergenreter with the Fort Dodge Fire Department. He explained that the fumigation process involves mixing two chemicals together which produces heat and a toxic gas - formaldyhyde - and is also flammable.

"It made it hazardous to enter the building," he said.

A few nearby homes were evacuated as a precaution, Anderson said. Stagecoach Road was closed for a short time to protect motorists from any potential hazardous fumes, according to Hergenreter.

"The amount of chemicals were not enough to affect the town of Woolstock," he said.

HOUSTON (KTRK) -- A chemical spill at St. Joseph's Hospital prompted officials to shut down its emergency room Monday !
The incident happened just after 8am. According to authorities, someone noticed a puddle inside a closet next the emergency room.

As a precaution, hospital officials shut down the ER and called the Houston Fire Department.

HFD officials determined about a half gallon of formaldehyde had leaked in the closet. They cleaned the mess and checked out three people who came in close contact with the chemical.

There is no word on what caused the spill.

MICHIGAN CITY | The accidental release of 1,500 gallons of a latex solution into a ditch leading to Trail Creek has been contained, a spokesman for the Indiana Department of Environmental Management said.
Barry Sneed, IDEM spokesman, said the release occurred around 3 p.m. Thursday due to an equipment failure at Fiber Bond Corp., 110 Menke Road.
Sneed said the material released contained 600 parts per billion formaldehyde, making it hazardous, but not toxic. Earlier reports that the material was primarily formaldehyde had caused concern.
Crews installed several dikes in the ditch, which kept the chemical contained, Sneed said. Heavy rains also helped in diluting the solution, he said.

A formaldehyde spill at Ashe Memorial Hospital at approximately 8 a.m. Wednesday morning necessessitated the activation of the Hospital's Hazardous Materials Response Plan, said R.D. Williams, Ashe Memorial Hospital's Chief Executive Officer.

"Due to the dangerous nature of this chemical, precautions have been taken to protect patients, visitors, and staff at Ashe Memorial," said Williams, in a press release at 1:12 p.m. Wednesday afternoon.

Staff had to be evacuated from a department at an Edinburgh hospital following a chemical spill.

A spokesman for Lothian and Borders Fire and Rescue Service said a man and a woman had been exposed to formaldehyde following an accident at the Western General Hospital.

The incident happened at 14:50 in the hospital's pathology department.

More than 30 firefighters were at the scene dealing with the chemical spillage.

Lothian and Borders Police said: "There is no concern for the public because it has been contained in the lab."

Jackie Sansbury, chief operating officer for NHS Lothian, said: "There was a minor chemical leak in the Pathology Unit of the Western General Hospital today.

ROYAL PALM BEACH, Fla. - The Palm Beach County Fire Rescue hazmat team is on scene at a "chemical spill" in Palms West Surgicenter located at 13001 Southern Boulevard in Royal Palm Beach.

According to Palm Beach County Fire Rescue's spokesman Don DeLucia, there was a chemical spill of formaldehyde in one of the labs of Palms West Medical Center.

Dispatchers received the call at approximately 2:30p.m.

"A portion of the Palms West Surgicenter has been evacuated while hazmat crews stabilize the situation.  One person suffered minor injuries and was taken to the emergency room of Palms West Hospital." said DeLucia.

ALGONQUIN – Emergency crews responded this morning to a small chemical spill at Jacobs High School in Algonquin.

District 300 spokeswoman Allison Strupeck said that about 300 students from 10 classrooms were moved to a different part of the school building after a teacher spilled less than a liter of formaldehyde in a hallway.

"Formaldehyde is not hazardous. It's not dangerous. But it is extremely odorous," Strupeck said. "The students in the surrounding classrooms were moved to another part of the school."

Algonquin - Lake in the Hills Fire Protection District personnel were called to the school at 8:35 a.m. regarding the spill, which was cleaned up by school staff. Due to the isolated area and the minimal amount that was spilled, no one was evacuated, the fire district said in a statement released about noon.

However, two school district staff members were transported to an area hospital for evaluation after they requested medical attention. Their condition, while not life threatening, is not known, the district release said.

The Milwaukee County Sheriff's Department initially responded to a hazmat spill at 4:29 on the campus of the Medical College of Wisconsin Loading Dock. The Wauwatosa Fire Department Hazmat Unit responded, and determined a 300 gallon container of formaldehyde ruptured.

PUNE: Leakage of a chemical kept in a box in the parcel office at Pune railway station, created panic among the workers. The incident happened on Thursday around 10.30 am. Few workers complained of minor irritation in the eyes following the leakage.

The Pune fire brigade officials rushed to the spot and removed the box to a safe place. The firemen also cleaned-up the parcel office.

Vijay Bhilare, station duty officer of Pune fire brigade, said that the box contained 6 kgs of the chemical, Formaldehyde. It had come from New Delhi for the Military hospital in Khadki.

Bhilare said, "We received a call around 10.30 am on Thursday regarding the leakage. When we went to the spot, the workers had already brought the box out of the parcel office."

Two people have been treated by paramedics after inhaling fumes from a chemical leak at a farm in Dorset.

The fire service was called by South Western Ambulance Service to Pentridge, near Cranborn, at about 1035 BST.

It is believed a container of formaldehyde was kicked over by an animal, a spokeswoman said.

Three fire crews have been dealing with the clean-up operation. It involved the chemical being absorbed into sand or soil.

A Highland village was cut off for several hours yesterday because of fears of a risk to health following reports of a major chemical spill.
Five fire appliances attended the incident with the brigade’s specialist environmental protection unit, which made an 80-mile dash from Inverness. The incident happened at around 12.30pm on the B863 road linking Kinlochleven with North Ballachulish following a report, believed to have been made by the driver, of a leak from drums aboard his large lorry.
The chemical leaking from the 5.5-gallon drums was quickly identified as a formaldehyde methanol solution, which is a hazardous material that can produce a burning sensation in the eyes, nose and throat.
A fire appliance from Kinlochleven was joined by two tenders from Fort William and a fire engine and the protection unit, with the specialist hazard materials officer from Inverness.

HARRISON TOWNSHIP, Ohio -- Fire crews responded to a local doctor’s office after a worker spilled a hazardous chemical.
The spill happened at the Primed Physician’s Office in the 7200 block of North Main Street in Harrison Township. One employee was taken to a local hospital after the spill.
Workers told News Center 7 that it smelled like a bio experiment that had gone wrong after two few gallons spilled inside the building.

Fire crews said a worker spilled formaldehyde in the building on Thursday morning.

A medical office building at Delnor Hospital in Geneva was evacuated Tuesday after a "minor" hazardous materials spill, firefighters said.
No injuries were reported in the incident, which happened around 12:19 p.m. at 351 Delnor Drive, when a nurse practitioner bumped into a container of formaldehyde, spilling about 2 ounces, hospital and fire officials said.
"Following the correct procedure, she left the room, put a towel under the door, evacuated the office and called the fire department," Deputy Fire Chief Jerry Koster said.
Hospital spokesman Brian Griffin said about 100 people were evacuated from 16 medical offices in the building.
Koster said people were allowed back in about two hours later, after a hazardous materials team cleaned up and investigated.
"It was a relatively minor spill," Koster said. "But until we get it assessed and know what we're dealing with, we err to the side of safety."

SACRAMENTO, Calif. -- Three people received medical treatment Monday morning after a chemical spilled at a building that is part of the UC Davis Medical Center complex, a fire official said.
The accident occurred at 2521 Stockton Blvd., said Capt. Jim Doucette of Sacramento Fire Deparmtent.
Doucette said an ounce or two of formaldehyde spilled, causing a handful of people to get dizzy. The victims were treated at the scene and were not taken to a hospital.
The spill occurred in a fourth-floor medical office.
The main UC Davis Medical Center hospital was not affected, Doucette said.
